Assessment of ownership of smart devices and the acceptability of digital health data sharing | npj Digital Medicine

Smart portable devices- smartphones and smartwatches- are rapidly being adopted by the general population, which has brought forward an opportunity to use the large volumes of physiological, behavioral, and activity data continuously being collected by these devices in naturalistic settings to perform research, monitor health, and track disease. While these data can serve to revolutionize health monitoring in research and clinical care, minimal research has been conducted to understand what motivates people to use these devices and their interest and comfort in sharing the data. In this study, we aimed to characterize the ownership and usage of smart devices among patients from an expansive academic health system in the southeastern US and understand their willingness to share data collected by the smart devices. We conducted an electronic survey of participants from an online patient advisory group around smart device ownership, usage, and data sharing. Out of the 3021 members of the online patient advisory group, 1368 (45%) responded to the survey, with 871 female (64%), 826 and 390 White (60%) and Black (29%) participants, respectively, and a slight majority (52%) age 58 and older. Most of the respondents (98%) owned a smartphone and the majority (59%) owned a wearable. In this population, people who identify as female, Hispanic, and Generation Z (age 18–25), and those completing higher education and having full-time employment, were most likely to own a wearable device compared to their demographic counterparts. 50% of smart device owners were willing to share and 32% would consider sharing their smart device data for research purposes. The type of activity data they are willing to share varies by gender, age, education, and employment. Findings from this study can be used to design both equitable and cost-effective digital health studies, leveraging personally-owned smartphones and wearables in representative populations, ultimately enabling the development of equitable digital health technologies.

Arterial stiffness assessment using PPG feature extraction and significance testing in an in vitro cardiovascular system | Scientific Reports

Cardiovascular diseases (CVDs) remain the leading cause of global mortality, therefore understanding arterial stiffness is essential to developing innovative technologies to detect, monitor and treat them. The ubiquitous spread of photoplethysmography (PPG), a completely non-invasive blood-volume sensing technology suitable for all ages, highlights immense potential for arterial stiffness assessment in the wider healthcare setting outside specialist clinics, for example during routine visits to a General Practitioner or even at home with the use of mobile and wearable health devices. This study employs a custom-manufactured in vitro cardiovascular system with vessels of varying stiffness to test the hypothesis that PPG signals may be used to detect and assess the level of arterial stiffness under controlled conditions. Analysis of various morphological features demonstrated significant (p < 0.05) correlations with vessel stiffness. Particularly, area related features were closely linked to stiffness in red PPG signals, while for infrared PPG signals the most correlated features were related to pulse-width. This study demonstrates the utility of custom vessels and in vitro investigations to work towards non-invasive cardiovascular assessment using PPG, a valuable tool with applications in clinical healthcare, wearable health devices and beyond.

Cet article présente une étude sur l'évaluation de la rigidité artérielle à l'aide de l'extraction des caractéristiques de la photopléthysmographie (PPG) et des tests de signification dans un système cardiovasculaire in vitro. Les chercheurs ont utilisé des vaisseaux sanguins artificiels avec différentes rigidités pour simuler des maladies vasculaires et ont mesuré les caractéristiques de la PPG pendant le flux pulsatile. Les résultats montrent que certaines caractéristiques de la PPG sont corrélées à la rigidité artérielle, ce qui suggère que la PPG pourrait être utilisée comme outil pour détecter des signes de maladies cardiovasculaires liées à la rigidité artérielle.

Cette recherche porte sur la façon dont le quantified-self peut changer le comportement quotidien de mobilité urbaine des usagers. Dans le cadre d’une démarche exploratoire, nous analysons cinquante-cinq entretiens individuels d’un public de jeunes utilisateurs quotidiens de smartphones. Nos résultats soulignent, par le biais de la théorie du cadrage, l’importance de la forme de la présentation de l’information auto-quantifiée dans le cadre d’une application à dimension environnementale. L’information CO2 d’une mobilité individuelle a plus d’impact si les bénéfices sont présentés de façon simple et en faible quantité (vs complexe), sous forme de gains (vs les pertes), dont la portée est individuelle (plutôt que collective), en privilégiant les normes descriptives (aux normes injonctives), avec dans une perspective court terme (vs long terme). Dans une démarche de transition écologique, des pistes de réflexion managériales sont proposées notamment sur la manière dont les praticiens peuvent concevoir une application mobile à visée environnementale.

Emerging sensing and modeling technologies for wearable and cuffless blood pressure monitoring | npj Digital Medicine

Cardiovascular diseases (CVDs) are a leading cause of death worldwide. For early diagnosis, intervention and management of CVDs, it is highly desirable to frequently monitor blood pressure (BP), a vital sign closely related to CVDs, during people’s daily life, including sleep time. Towards this end, wearable and cuffless BP extraction methods have been extensively researched in recent years as part of the mobile healthcare initiative. This review focuses on the enabling technologies for wearable and cuffless BP monitoring platforms, covering both the emerging flexible sensor designs and BP extraction algorithms. Based on the signal type, the sensing devices are classified into electrical, optical, and mechanical sensors, and the state-of-the-art material choices, fabrication methods, and performances of each type of sensor are briefly reviewed. In the model part of the review, contemporary algorithmic BP estimation methods for beat-to-beat BP measurements and continuous BP waveform extraction are introduced. Mainstream approaches, such as pulse transit time-based analytical models and machine learning methods, are compared in terms of their input modalities, features, implementation algorithms, and performances. The review sheds light on the interdisciplinary research opportunities to combine the latest innovations in the sensor and signal processing research fields to achieve a new generation of cuffless BP measurement devices with improved wearability, reliability, and accuracy.
